EAST LANSING, Mich. Often faced with overwhelming anxiety, patients newly diagnosed with lung cancer can find themselves in distress, and new research recommends nurses play a key role in alleviating concerns, leading to a better quality of life for patients.

A diagnosis of lung cancer the leading cause of cancer death in the United States brings with it high levels of stress and raises existential issues and death-related thoughts and concerns in patients, said Rebecca H. Lehto, assistant professor in the College of Nursing at Michigan State University.

In a paper published in the current edition of the Clinical Journal of Oncology Nursing, Lehto lays out several ways nurses can help bridge the gap between diagnosis and a strong psychological outlook during treatment.

"People facing life-threatening diagnoses often feel alone, and newly diagnosed lung cancer patients with early stage disease can have concerns about an uncertain future, the potential for treatment failure, the cancer spreading and the possibility of death," she said. "Health care providers, who are uncomfortable with and avoid discussing end-of-life questions, may contribute to a patient's alienation.

"Nurses who are comfortable with listening for and discussing existentially related concerns may be in a better position to promote the patient's psychological adaptation."

Too often, health care providers are solely focused on treatment and disease management, Lehto said. Her research identifies the types of death-related concerns that people experience and strategies to help resolve the normal but often distressing psychological responses in the early post-diagnostic and treatment period.

"With a nurse's help, if a patient can resolve the issues that impact quality of life during early treatment, it may greatly improve their outlook," she said. "It is difficult for patients to focus on learning about anti-cancer treatment if they are overwhelmed with anxiety and distressing thoughts."

Lehto said the biggest hurdle to overcome often is helping patients come to grips with the impact of the disease and the possibility of death on loved ones.

"Nurses first of all need to be a supportive, nonjudgmental presence," she said. "Patients are making life decisions and sometimes are making death preparations. Nurses can provide patient education materials and offer support in managing life affairs."

Other strategies include exploring life stories, discussing personal relationships and spiritual resources and helping patients locate resources to make funeral arrangements, update wills and other legal issues. Nurses can listen for patient concerns and focus only on personally relevant information so as not to further overwhelm them.

Lehto said the nurse role is essential despite concern that such discussions will increase psychological distress for patients.

"By addressing death-related concerns early, nurses will determine individualized needs for mental health or other supportive interventions or services," she said.

FORT MYERS, Fla. - Bobby Valentine has only been wearing a Boston Red Sox uniform for about two weeks and he's already taking a few jabs at the archrival New York Yankees.

Many thought that the outspoken manager's arrival in Boston could add even more spice to one of the most heated rivalries in professional sports, and it hasn't taken long for that to hold true.

In one fell swoop on Tuesday, Valentine said Derek Jeter didn't need to make the flip-to-home relay that stands as one of the most celebrated plays in Yankees lore and fondly recalled when retired Red Sox catcher Jason Varitek "beat up Alex" Rodriguez in 2004.

The Red Sox were working on relay throws at spring training on Tuesday and Valentine was asked about one of the most famous relay plays in all of baseball in Game 3 of the 2001 ALDS between the Yankees and Athletics.

Down 2-0 in the series and with a 1-0 lead in the seventh inning, Yankees right fielder Shane Spencer missed the cutoff man on a double by Terrence Long. That's when Derek Jeter seemingly came out of nowhere to grab the overthrow in foul territory near the first baseline and flipped the ball home to get Jeremy Giambi at the plate.

The Yankees hung on in the game, and rallied to win the series.

"We'll never practice that," Valentine said. "And I think (Jeter) was out of position and I think the ball gets (Giambi) out if he doesn't touch it, personally."

Most observers credit Jeter's hustle and instincts with turning around the series in the Yankees' favor. And several Yankees have said that Jeter would practice that play in spring training, which drew a scoff from Valentine.

"That was amazing that he was there," Valentine said. "Then it was more amazing to say they practiced it. I don't believe it."

But Valentine wasn't finished there.

Even though he is just starting to put his finger prints on the organization, Valentine was asked to comment on Varitek's 15 seasons in Boston and what he meant to the organization. Like many others, Valentine used Varitek's confrontation with Rodriguez in a July 2004 game to sum up his mentality and his demeanor.

Red Sox pitcher Bronson Arroyo drew Rodriguez's ire when he plunked A-Rod in the game. After Rodriguez started cursing at Arroyo, Varitek came to his pitcher's defense by shoving his catcher's mitt into the third baseman's face, sparking the benches to clear. The Sox came back and won the game 11-10 and went on to win the World Series that season.

Valentine was nowhere near Fenway Park that day, but that didn't stop him from giving the decision to Varitek.

"He is a man's man," Valentine said of Varitek. "He was a big hitter when needed. He was a leader of the pitching staff. He was able to beat up Alex. All that stuff is good stuff. He was exactly what he was supposed to be."

The rivalry will be renewed when the two teams meet in spring training on March 13. Their first series of the regular season will begin April 20 at Fenway.

87-Year-Old Aiken County Man Says He's Lucky to Be Alive After Friday Tornado

In a rural area, not far from the town of Wagener, clean-up is underway.

Here, Friday?s storm snapped trees like they were toothpicks, and tore through 87-year-old Virbraschi Aversa?s home like it was nothing.

?The destruction is terrible and the destruction is fast,? said Aversa.

The tornado ripped through the front door, which it still bolted and locked. It ripped the roof off more than half of the house and destroyed everything inside, but luckily, Vibraschi was in the only room where the ceiling didn?t cave in. He says he was inside the kitchen unloading groceries when it hit.

?Any other room, like I said and I would be dead,? he said.

As Verbrashi sifts through the rubble, picking up whatever he can salvage of his home, he decides he is lucky. ?I?m blessed,? he said.

His neighbors, friends, and family help to clean up much of the debris, but there is still a lot of work to be done.

Sandler sets Razzie record: 11 worst-movie noms (omg!)

SANTA MONICA, California (AP) ? When Adam Sandler's bad, he's really bad, according to voters for the Razzies, an Academy Awards spoof that singles out the worst movies of the year.

Sandler received a record 11 nominations Saturday for the Razzies as star, producer or writer on three 2011 movies ? "Jack and Jill," ''Just Go with It" and "Bucky Larson: Born to Be a Star."

That more than doubled the previous record of five Razzie nominations held by Eddie Murphy for 2007's "Norbit."

Sandler's nominations include worst actor for both "Jack and Jill" and "Just Go with It" ? and worst actress for "Jack and Jill," in which he plays a family man and his own twin sister.

Sandler also had two nominations as worst screen couple opposite Jennifer Aniston or Brooklyn Decker in "Just Go with It" and opposite Katie Holmes, Al Pacino or himself in "Jack and Jill."

As a producer, Sandler was credited with worst-picture and worst prequel, remake, rip-off or sequel nominations for both "Bucky Larson" and "Jack and Jill." He also shared in worst-screenplay nominations as a writer on both movies, and a worst-ensemble nomination for "Jack and Jill."

"It's almost karmic for someone to have made that much razz-able stuff in one year," said Razzies founder John Wilson. "He has angered someone really powerful, I would say."

Along with "Bucky Larson" and "Jack and Jill," worst-picture contenders are "New Year's Eve," ''Transformers: Dark of the Moon" and "The Twilight Saga: Breaking Dawn ? Part 1."

"Jack and Jill" led with 12 nominations, with "Transformers" second with nine and "Breaking Dawn" right behind with eight.

Razzie nominations were released on the eve of the Oscars. Winners will be announced on April Fool's Day.

For worst actor, Sandler is up against "Bucky Larson" star Nick Swardson, along with Russell Brand for "Arthur"; Nicolas Cage for "Drive Angry," ''Season of the Witch" and "Trespass"; and Taylor Lautner for "Abduction" and "Breaking Dawn."

Swardson also was nominated for worst supporting actor for "Jack and Jill" and "Just Go with It."

For worst actress, Sandler is joined by another cross-dressing actor, Martin Lawrence in "Big Mommas: Like Father, Like Son." Also nominated were Sarah Palin for her documentary "The Undefeated"; Sarah Jessica Parker for "I Don't Know How She Does It" and "New Year's Eve"; and Kristen Stewart for "Breaking Dawn."

Several Oscar winners made the Razzies list, among them Cage (a best-actor winner for "Leaving Las Vegas") and Pacino (a best-actor winner for "Scent of a Woman"), who was nominated for worst supporting actor for "Jack and Jill."

Bill Condon, an Oscar winner for his "Gods and Monsters" screenplay, was among the worst-director nominees at the Razzies for "Breaking Dawn."

The group traditionally has announced its nominees the day before the Oscar nominations and its winners the night before the Oscars. But Wilson and the roughly 600 voting members for the Razzies decided to experiment with an April 1 date this time, figuring April Fool's Day was an ideal time for their mock ceremony.

"I have always wondered if we stepped slightly away from lockstep with the Oscars, what would happen," Wilson said. "We just kept hoping the Oscars would do their ceremony on April 2, but they just never did."

Acute demand for US geoscientists prompts call for higher ed action

Boulder, CO, USA A recent American Geosciences Institute (AGI) workforce evaluation estimates that by 2021, 150,000 to 220,000 geoscience jobs will need to be filled. The AGI report notes that at current graduation rates, most of these jobs will not be able to be filled by U.S. citizens.

Citing great concern about the acute need for well-trained, well-educated geoscience graduates to fill the geoscience workforce, Geological Society of America President John Geissman is calling for colleges and universities to recognize the value of strong, adequately supported geoscience departments. High-quality geoscience education, Geissman notes, is essential to understanding and adequately addressing the "very pressing needs of our society," including sustainability and human-caused climate change, as well as keeping the growing number of geoscience jobs filled by U.S. citizens.

Included in his call for action, Geissman refers to two very recently approved GSA Position Statements that focus on the importance of teaching earth science and expanding and improving geoscience education in institutions of higher learning.

Both position statements are online at www.geosociety.org/positions/. All GSA position statements include suggestions for how to implement and support the call to action.

"The Importance of Teaching Earth Science" recognizes that basic knowledge of earth science is essential to meeting the environmental challenges and natural resource limitations of the twenty-first century and notes that earth-science education should begin at the K? level and include advanced classes led by well-qualified science teachers.

"Expanding and Improving Geoscience in Higher Education" calls specifically for robust, well-supported geoscience departments not only to ensure an increase in the number of geoscience students available to the workforce but also to provide the training necessary "to address crucial societal issues that have the potential to impact global economic security and the well-being of human populations" across the globe.

Fashion Week gowns take glamour cues from bygone eras (Reuters)

NEW YORK (Reuters) ? Elegance, sophistication and a touch of mystery dominated the runway at New York Fashion Week as designers took cues from bygone eras to feature glamorous gowns in silk, brocades, lace and velvet in their 2012 Fall/Winter collections.

Like the Hollywood hit films "The Artist" and "Midnight in Paris" and the hugely popular TV drama "Downton Abbey" that captured an earlier age, designers created sleek, sultry styles with deep-cut fronts and backs with beads, embroidery and pleats reminiscent of the 1920s, '30s and '40s.

For Los Angeles-based, Japanese-born Tadashi Shoji it was 1930s Shanghai that captured his imagination. Graceful silhouettes, column gowns, drop waists, embroidered lace, handkerchief hems and cap sleeves from the Golden Age of Shanghai, when the city was known as the "Paris of the Orient," were recreated on the catwalk.

"It was a very opulent, mysterious, moody period in Shanghai and I wanted to capture it," he said after his show.

A flame-colored, washed velvet long-sleeve gown featured a beaded lace cowl back, while another hazel digital peony print hammered satin gown had an open back with hand-beaded detail.

Blouson dresses, some in tea length, and floral embroidered tulle and lace were also prominent, as was the color gold.

"Everything is gold, but it is not a flashy gold. We did a very subtle, tarnished gold. I think gold is in this season," he said.

GLAMOUR, GRANDEUR AND FEMME FATAL

Asian influences were also evident in Zac Posen's collection with its geisha-look hair and makeup, kimono and origami-like details and figure-hugging gowns in gold, red, green and blue.

Dennis Basso chose "Mystery on the Orient Express" as the theme for his show with its palette of taupe, black, gray, white, navy and cayenne and luxurious furs.

"When you just say that line it sets a tone in everybody's head -- a touch of glamour and grandeur from a bygone era. I've taken a lot of those evening looks and translated them for the modern woman," he said.

Cocktail dresses and evening gowns were adorned with hand-beaded Swarovski crystal and small beads. He matched a black lavishly embroidered silk tulle dress with a black quilted Russian ermine and sable jacket and a sapphire and silver embroidered, trailing gown with a sapphire Finn raccoon vest.

"I think of that period in the late '30s of women getting dressed for dinner on the Orient Express, crossing the Atlantic or going to the Colony Club restaurant in New York or the Stork Club and bringing it forward, making it modern, making it sexy, making it today -- using the vintage aspect as a touch of inspiration, not necessarily, which is important, creating a vintage look," Basso explained.

Jenny Packman, a favorite of the Duchess of Cornwall, was drawn to the femme fatale look with a collection based on film noir with edgy, sultry creations in red, silver, blue, cream and black.

Marchesa's Georgina Chapman noted that '20s and '30s styles were "in the air" but the designer whose creations feature prominently on the red carpet at Hollywood award shows said she was inspired by a painting.

Her collection, in black, white, reds and blues, is based on the 1878 work "A Soul Brought to Heaven," by William Adolphe Bouguereau, which depicts two angels carrying a young woman through dark clouds.

"We are looking at the idea of death and angels, religious aspects of iconography and religious art," Chapman explained in an interview.

Tulle, lace, beading, embroidered bodices, feathers and dramatic flared and full hand-draped skirts were all part of the ultra-feminine collection.

A gold strapless gown conveyed a sense of old Hollywood glamour and was a striking contrast to a nude feather embroidered illusion gown with a cascade skirt.

Like other designers showing at New York Fashion Week, Chapman said the economy has sharpened the focus of her creations.

"It's been interesting with the economy, and it has made me and a lot of designers focus very much on what they are doing," she said. "You really have to give a consumer a reason to part with their money ... to give them something they can't find elsewhere."

Rather than making quick, impulse purchase of something that may only be worn once, Basso said today's women are making wiser choices and building their wardrobe on well-made clothes that can be worn again and again.

"Women will always want to look beautiful and I think that their partners enjoy that too," said Chapman.

Strategic Planning ? Counselling Connect

In the last Blog, I discussed a ?Needs Analyses?.? The objective was to point out a comparison between being salaried versus self-employed. For example, it will take 545 clinical hours per year or 10 counselling hours per week, to achieve a similar gross salary that you would receive if employed by an agency. I felt it was important to provide a conceptual framework from which to work and answer the questions: Is starting a private practice in clinical counselling financially doable given your location, financial resources, and your market place?

In order to guide you through this process, I will be disusing a strategic planning, operational planning, budgeting, and forecasting. [i] ??

Starting with strategic planning:

What does your business do? It provides clinical counselling services. The key word is ?services?. You provide a service, which is intangible.? Furthermore, clinical counselling services are not as much an essential service, such as seeing your dentist, doctor, or optometrist on a regular basis. ?In addition, there is a stigma attached to mental health issues, which make offering counselling services a harder sell.

Who does your business service?

?You need to assess your geographic location, the competition, the culture, and the economy of where you wish to locate your private practice. ?As well, you need to define your client base, which I assume would correlate to your specialty.? For example, if you wish to limit your practice to children and teens then you need to check the number of schools in your area.

Examples of some of the places where adventure team building can take place are resorts, conference centers, and parks. And many organizations can custom-build a corporate adventure for you that is designed to develop the specific skills required by your employees.

Adventure team building is also an excellent way to celebrate a success story and reward your team for a job well done. What this does is help to continue the success, boost future team performance and increase morale. When everyone in the office feels motivated, energized and excited about work, more success is definitely on the horizon regardless if you choose a helicopter ride or a camping experience in the woods.
